Two opposite problems, a few hours apart
Warmth before a feed encourages the let-down reflex and softens a full breast, which makes it easier for a baby to latch onto and easier for milk to move. It's the same reason a warm shower often gets things going when nothing else has.
Cold afterwards does the opposite job. When milk first comes in, usually around day three, breasts can become hard, hot and genuinely painful. Cold reduces swelling and numbs the ache, which is why it's the standard suggestion for engorgement.
These are old, well-established comfort measures rather than treatments, and both are routinely recommended. What makes them annoying in practice is the equipment: a hot flannel goes cold in ninety seconds, and a bag of frozen peas sits on one spot and soaks your shirt. A shaped gel pack simply holds the temperature and the shape long enough to be useful.

What makes a gel pack worth buying
- A shape that stays put: curved with a cut-out for the nipple, so it wraps the breast rather than balancing on it. You can lie back without it sliding off.
- Covers that wash: a soft cloth sleeve on each pack, removable and machine washable. Gel straight against skin is either too cold or too hot, and this is the layer that stops that.
- Usable while pumping: the cut-out means a pack can stay in place around a pump flange, which is when warmth is most useful.

Warm before, cold after
- Before a feed: warm a pack in short bursts, test it on the inside of your wrist, slide it into its cover and hold it against the breast for a few minutes.
- Feed or pump as usual. If you're pumping, a warmed pack can stay on around the flange.
- After a feed: take a chilled pack from the fridge, put it in its cover and rest it on for ten to fifteen minutes.
- Rinse the covers when they need it, and put the used pack back in the fridge ready for next time.
Never put a warmed pack against skin without its cover, and never warm it for longer than the pack says. Overheated gel burns, and you won't feel it as quickly as you'd expect on skin that's already sore.
Comfort, not treatment, and a burn risk if rushed
These ease engorgement and help with let-down. They do not treat mastitis, blocked ducts that aren't shifting, or an abscess.
Please call a doctor if part of your breast is hard, red and hot and you feel shivery or feverish, if a lump doesn't move after a couple of days, or if you feel generally unwell with it. Mastitis is common and very treatable, and it gets worse if it's left.
Always use the cloth cover, always test a warmed pack on your wrist first, and don't use heat on skin that's broken. Sore, swollen skin is less sensitive than usual, which is exactly how people get burnt.
